2. TRANSFER CREDENTIALS / OTR COPY FOR KEYSTONE COLLEGE / HONORABLE DISMISSAL (from previous or last school attended) – For transferees
3. 3 COPIES OF 2X2/PASSPORT SIZE RECENT ID PICTURE ( white background)
4. ORIGINAL/AUTHENTICATED PSA BIRTH CERTIFICATE
5. ORIGINAL/AUTHENTICATED PSA MARRIAGE CERTIFICATE (if applicable)
